Authorities have not yet determined the cause of the accident, though deteriorating sea conditions and shoaling at the bar had been previously reported. The Australian Maritime Safety Authority has launched an investigation, which will examine both the sea state and potential mechanical issues with the boat.

It's still not exactly clear how or why, but the charter boat flipped just after 5am. The four tourists managed to swim to shore. The boat washed up on Mooloola Bar Beach, but the skipper, Robert Smith, died. Today, he's been remembered as a veteran of Sunshine Coast Seas, a hard-working father, a husband and a friend. Initially, it was feared he was still trapped inside the crumpled boat. A number of people tried to get inside, but a few hours later, the 52-year-old was found at sea. At the moment, it's unclear whether there was a rogue wave or particularly treacherous conditions this morning, but the Maritime Safety Authority did warn just yesterday about shoaling at the bar. The Australian Maritime Safety Authority investigation will obviously investigate what part that played in the tragedy. The fishermen have been talking about the bad conditions for a long time, so obviously something's got to be done now, because there's been a death. Now Taylor, the investigation will also look at whether there were mechanical issues with the boat, but for now it's understood the main focus will be deteriorating sea state conditions on the bar.
