Witnesses reported mass killings, widespread injuries from rocket attacks, and severe malnutrition as food supplies were cut off, forcing many to eat animal feed.

Aid organizations such as Doctors Without Borders are treating trauma and malnutrition cases among those who reached safety, but resources are limited and many injuries have become infected. The International Organization for Migration estimates that over 60,000 people have fled al-Fashir, though their whereabouts are unknown, and up to 200,000 civilians may still be trapped in the city.

The city was the final stronghold of the Sudanese army in the western Darfur region before it fell to the paramilitary group after an 18-month siege. Witnesses reported mass killings following the RSF's advance, and many of the city's residents remain unaccounted for. Abdallah Hassaballah is among the thousands who arrived in the town of Tawila after fleeing the RSF takeover in al-Fashir. Once you leave the gates, the bodies start. The bodies continue all the way to Gari. Some were killed by thirst, some by exhaustion, some by their injuries, the bleeding. Some were injured by the rockets in al-Fashir. They hurt more than gunshots, the shrapnel. They get into your body. They cause your legs to swell, stop your blood flowing. It exhausts you. It impacts you within two or three hours. International aid agency Doctors Without Borders runs the clinic, which is treating the wounded and malnourished. Mouna Hanibali is a medical team leader there. The main cases we receive are trauma cases, primarily gunshot wounds, unstable fractures, infected wounds and surgical complications that occurred in al-Fashir. But given the lack of equipment and medicines, such as antibiotics, most of these injuries become infected and require a second operation. Al-Fashir's capture marked a turning point in a two-and-a-half-year-old war between the RSF and the army. The siege had cut off food supplies, forcing many locals to eat animal feed as they sought shelter from drones and shelling. Hanibali again. One of our main fears concerns all the civilians still trapped in al-Fashir. Right now, the situation of those who are arriving is very worrying. Malnutrition, lack of medical care and a lack of all basic necessities, starting with water and food. In addition to those who reached Tawila, the International Organization for Migration believes more than 60,000 others have escaped al-Fashir, though their whereabouts remain unclear. Estimates say as many as 200,000 people may still be trapped in the city.
