A newly released government report has found that Ashley Paul Griffith, described as Queensland's worst pedophile, was able to abuse children for over 13 years due to repeated failures by authorities and employers to act on numerous warnings and complaints. The report, authored by Luke Twyford and titled 'Caught in Plain Sight,' documents 18 missed opportunities to stop Griffith and 13 enabling events that allowed his offending to continue.

The report recommends 28 measures, including a mandatory reportable conduct scheme for organizations, to prevent similar failures in the future. The Queensland government has committed to introducing this scheme, while legal representatives for victims' families are calling for additional safety measures such as banning personal mobile devices in child care centres.

The report is caught in plain sight because too often those that seek to abuse children are standing right in front of us. The report authored by Luke Twyford found Ashley Paul Griffith could have been stopped on 18 separate occasions. And 13 events that enabled him to continue to offend. Reports went back as far as 2009 where one child called Griffith the mean man after a nappy change. Parents said following the incident their child suffered from separation anxiety and refused to go to the toilet. The parent in that case also complained Griffith took photos of children without parental permission. Police interviewed the child involved, the complaint was then filed but not actioned further. The former child care worker would go on to abuse for another 13 years before he was caught. Colleagues and centre managers spoke up about him. He was dismissed from his employment on more than five occasions. He had police investigations into his conduct. He had early child care regulatory investigations into his conduct and yet none of that made a difference. The report put forward 28 recommendations including a reportable conduct scheme which will create a legal obligation for companies to report misconduct against children.
