The Australian Transport Safety Bureau has released a preliminary report into a crash at Avalon Airshow, which left pilot Glenn Collins seriously injured. It found the use of pyrotechnics in the show hampered the emergency response.

A preliminary report from the Australian Transport Safety Bureau (ATSB) has been released following an airshow crash at Avalon Airport involving the Sky Aces. During a routine, pilot Glenn Collins was seriously injured when his aircraft failed to recover from a manoeuvre and collided with the ground. 

The ATSB found no mechanical faults with the aircraft but noted differences between the pilot's training and the manoeuvre executed during the event, including lower altitudes than previously practised. The investigation also identified challenges in emergency response due to restricted access for rescue services. The ATSB will issue safety recommendations in its final report.

The Sky Aces putting on a show at Avalon Airport. During the routine, the pilot began a triple avalanche manoeuvre and entered the aircraft into a loop. But the spectacle didn't last. The aircraft momentarily returned to stable flight while still inverted. It then entered the back half of the loop. However, the aircraft's descent rate was unable to be arrested before it collided with terrain. Leaving pilot Glenn Collins seriously hurt. More than three months on, the Australian Transport Safety Bureau has released its preliminary report. Now, we've certainly found no mechanical faults or defects that are likely to have led to the accident. Investigators have also released footage from the day, some from a GoPro inside the cockpit. We did find differences between what had been trained for and what had been practised versus what was executed on the day. They noticed the aircraft's altitude during the performance was sometimes lower than during the pilot's previous training, but the ATSB won't say if the pilot was to blame. We don't rule in or out anything. They're also investigating challenges reported by the Aviation Rescue Firefighting Service. As pyrotechnicians were attending to the pilot, they were unable to safely guide the Aviation Rescue Firefighting Service through the pyrotechnician's area, which increased their response time. Once the investigation is complete, the ATSB will make recommendations to improve safety within the sector in its final report. Ashlet Konowski, 10 News.
