A detailed inquest into the Bondi Junction stabbing attack, which resulted in six deaths and 16 injuries, has produced a 670-page report for the coroner. The council assisting the coroner made 26 recommendations after consulting with psychiatric experts, focusing on improving mental health care, security procedures, and media reporting.

The inquest highlighted significant errors in the treatment of the attacker, Joel Kauchi, including the decision to wean him off medication without adequate risk assessment. Security lapses, such as slow alarm responses and insufficient control room staffing, were also noted. The coroner is expected to deliver final findings by the end of the year.

It was a violent, fast-moving scene. Now, learning how to prevent such a tragedy is a lengthy, meticulous process. 670 pages have been prepared for the coroner in response to the Bondi Junction stabbing attack, which injured 16 victims and took six lives. Council assisting argued the response from the security control room operator was too slow. Peggy Dwyer said it was not an attack on her personality. The point is she wasn't competent to respond on the 13th of April. She shouldn't have been in that control room on her own, however rare an active armed offender is. Further recommendations include supported accommodation for people with severe mental illness, recognition for brave first responders, and greater sensitivity in media reporting.
