The National Transportation Safety Board (NTSB) reported that the Critical Airport Surface Detection System (ASDX) failed to activate before a fatal collision between a plane and a fire truck at LaGuardia Airport. Investigators noted that the fire trucks involved did not have transponders to provide precise location data to air traffic controllers, a measure recommended but not required by the FAA. 

The incident occurred after air traffic control cleared the fire truck to cross the runway when the plane was just 100 feet above ground. Ten seconds later, an urgent stop warning was issued, but it was too late to prevent the crash, which resulted in the deaths of two pilots. The NTSB also highlighted longstanding concerns among controllers regarding ground safety systems.
