The investigation found that bolts securing the door plug were removed and not replaced, with inadequate training, documentation, and oversight contributing to the error. The NTSB noted that the team responsible for removing the door panel lacked experience and failed to complete necessary paperwork, which would have triggered an inspection to catch the missing bolts. Additionally, the Federal Aviation Administration (FAA) was found to have insufficiently monitored recurring issues at Boeing, failing to identify broader systemic problems. The incident resulted in a rapid loss of cabin pressure and scattered objects from the aircraft, but no injuries were reported.
